Employee Satisfaction Survey: Write Questions and Read Aggregate Results
An employee satisfaction survey needs a clear purpose, answerable questions and an honest explanation of how responses will be handled. This original practice questionnaire concerns recent work processes. It is not a validated instrument, and no real employee responses are collected here.

Fictional purpose: understand employees’ recent experience of materials, priorities and handovers. The practice invitation covers the past four weeks, says participation is optional and asks people not to include names or personal case details in free text. Before a real launch, the survey owner must state who can access raw responses, what will be reported and how records will be managed.
Do not promise anonymity merely because the form omits a name field. Platform logs, small groups and identifying comments can affect identifiability. In this example, reporting is aggregate only and no subgroup or quotation is published; those design choices are not a universal privacy guarantee.
Use one topic per item
Pew Research Center explains why clear wording and single-concept questions matter. Replace “How happy are you with our excellent resources and supportive manager?” with separate neutral questions that people can answer about their work.
Original short questionnaire
How satisfied were you with access to the materials needed for your assigned work?
How satisfied were you with the clarity of your assigned work priorities?
How satisfied were you with the handover information you received?
Optional: What one work process would you most like clarified? Do not include names or personal case details.
For Q1–Q3: very dissatisfied; dissatisfied; neither satisfied nor dissatisfied; satisfied; very satisfied; not applicable; prefer not to answer. The numbered categories run from 1 to 5 only for the five satisfaction choices.
Calculate participation and the item denominator
Fictional invitations: 20 eligible employees; 12 submitted a response. Response rate is 12 ÷ 20 × 100 = 60%. Eight did not respond. For Q1, the 12 submissions contain: one very dissatisfied, two dissatisfied, three neither, four satisfied, one very satisfied and one not applicable.
Under the stated “satisfied or very satisfied” grouping, five of 11 substantive Q1 answers are favorable: 5 ÷ 11 × 100 ≈ 45.45%. The not-applicable response is reported separately and excluded from that denominator. Do not divide by 12 without changing the definition.
Preserve the missing-response limits
The eight nonrespondents did not supply opinions. Do not count them as satisfied, dissatisfied or neutral. Likewise, the Q1 result describes the 11 people who supplied a substantive answer, not every invited employee. A 60% response rate alone does not prove representativeness.
No Q2 or Q3 results have been supplied, so this fixture cannot calculate an overall satisfaction score. Combining distinct items would also require a stated rationale rather than assuming they measure one construct.
Prepare a transparent summary
Completed note: “Twelve of 20 invited employees responded. For materials access, five of 11 substantive answers were satisfied or very satisfied; one response was not applicable. Nonresponse and the small aggregate set limit interpretation. No subgroup or identifiable comments are reported.”
Use a small pilot to check whether the intended audience understands the wording and choices before a real survey. If a later wave changes the question or timeframe, document that change before treating its result as a direct trend.
Questions, answered
Does no name field guarantee anonymity?
No. Explain the actual collection and reporting process; do not make a guarantee the process cannot support.
Should not-applicable answers become neutral scores?
No. They have a different meaning. Keep the denominator and exclusions explicit.
