Employee Review Template: A Completed Evidence-Based Manager Record
An employee review template should help a manager explain the evidence behind a periodic assessment. The completed fictional record below keeps the work standard, observations and employee response separate. It is a practice document, not an official rating form or a disciplinary procedure.

Avery’s fictional catalogue role requires using an approved source, checking its version and flagging unresolved questions before the assigned deadline. The manager should evaluate those agreed tasks rather than add a new standard after seeing the work.
OPM’s performance-management cycle distinguishes planning, monitoring, development and rating. This example uses that general distinction to organize a periodic manager record; it does not import federal procedures or rating requirements into another employer.

Manager-owned catalogue review
Employee: fictional Avery Lane | Manager: fictional Noor Reed | Review period: May–June | Discussion date: July 2
Agreed work standard: update assigned catalogue entries from the approved source, complete the source/version check and flag unresolved items before the stated deadline.
Observation 1: May 8 record C14 used approved source v3 and included the completed version check before its deadline.
Observation 2: May 15 record C19 initially used superseded source v2. Avery corrected it to approved v3 after Noor identified the mismatch; the corrected entry met the revised deadline.
Manager summary: the records show one correctly checked update and one source-version error followed by a correction. These two observations do not establish an error rate for the entire period.
Employee response: Avery says both source versions remained in the working folder and requests clearer identification of the approved source. This explains a reported condition; it does not remove the need to check the source.
Agreed next step: Noor will ask the catalogue owner to confirm the approved-source location. Avery will use that source and record the version on the next three assigned updates.
Next check-in: July 16. Review the three completed records and any unresolved source questions. Do not infer completion before the work occurs.
Employee comments and corrections: recorded separately from the manager summary; unresolved factual differences remain visible.
Inspect the language of the assessment
“Used superseded source v2 on C19” is a dated observation. “Does not care about accuracy” is an unsupported interpretation of motivation. The correction also belongs in the record; removing it would give an incomplete account of what happened.
Do not turn two observations into a whole-period percentage. The fixture does not give the total number of updates or a representative sample. The manager can discuss the documented source-version issue without inventing a failure rate or an overall numerical rating.
Give the employee response a clear place
Avery’s account about the folder may identify a support problem for the catalogue owner to investigate. Keep it attributed as the employee’s report until checked. Record a factual correction if a source or date was wrong; retain a genuine disagreement rather than rewriting it as agreement.
The next step has two owners: Noor seeks confirmation of the source location, and Avery records the version used on new updates. A review that assigns every action to the employee would hide the manager’s support responsibility.
Reuse the editable fields carefully
The download includes the finished record and a blank field sequence. Replace the fictional names, dates and evidence with verified work records, then follow the organization’s approved review process. Keep access appropriate to the document rather than treating it as a public recognition message.
At the next check-in, review the new work that actually exists. If fewer than three updates have been assigned, record that limit and revise the check rather than marking the action successful by default.
Questions, answered
Is this the employee’s self-appraisal?
No. This document contains the manager’s evidence summary and a distinct employee-response section. The employee’s own reflection has a separate purpose.
Should a template assign a rating automatically?
This one does not. A formal rating needs the relevant organization’s standards and process, not a score invented from two example records.
