Workplace communication
Communication Breakdown: Trace a Divergent Request and Repair the Record
A communication breakdown can occur when different people act on different versions of a request. Repair starts by reconstructing the available record: what each version actually said, what changed and what remains unknown. This exercise investigates a completed message trail rather than practicing a proactive read-back script.
Read all four versions before assigning a cause
A fictional exhibition team is preparing label cards. The coordinator, designer and printer each have a record, but their wording diverges. The exhibit naming, quantities and timestamps below are supplied facts.
Original request trail
09:00 Approved brief v1: “Print 40 blue labels for Cedar exhibit; review a proof before production.”
09:20 Forwarded message: “Print 40 labels for Cedar; blue proof attached.”
10:00 Production sheet: “40 labels, blue proof approved.”
10:30 Printer note: “40 white labels produced; used standard white stock. No blue-stock instruction on production sheet.”
Find the two distinct changes
The 09:20 message no longer states that the labels themselves must be blue; “blue proof” could refer to the attachment rather than the stock. The 10:00 sheet also introduces “approved,” while the supplied earlier brief only requires review. No approval record is present in this trail.
The 10:30 note explains the printer’s stated basis, but it does not establish who wrote each intermediate version or why. Do not turn an unsupported approval into a claim that a named colleague deliberately misled the printer. Ask for the missing record and preserve the distinction between observation and inference.
Separate repair from a production decision
Under the approved brief, the intended quantity remains 40, the exhibit remains Cedar and proof review is still required. The trail does not supply authorization for a reprint, disposal of the white labels or a new cost. Correcting the communication record does not grant those decisions.
The communication-process framework distinguishes sent wording, interpretation and feedback. Here the practical evidence is the actual version trail. A generally “unclear communication” label is less useful than naming the lost color instruction and unsupported approval status.
Write a complete replacement source-of-truth update
This update carries the known requirement, observed deviation, missing decision, owner and superseded versions together. It does not falsely say that the printer has stopped or that a reprint is already approved. Those would need actual confirmation.
Original repair update
“Cedar label request — correction issued 10:45. Approved brief v1 requires 40 blue labels and proof review before production. The later production sheet’s ‘proof approved’ status has no supporting approval in the available record. Forty white labels are reported produced; do not treat them as an approved substitution. Coordinator owns confirming the required stock and obtaining a recorded proof decision. Printer: please report the current job status and any action awaiting authorization. This correction supersedes the 09:20 wording and 10:00 approval status; keep both earlier records for traceability.”
Check whether the repair reached the active records
Identify who still uses the production sheet, update its status through the agreed process and link it to the correction. A corrected chat message alone leaves the old sheet available for further use.
For a practice variation, add a verified 09:40 proof approval that covers layout only. Decide what that evidence resolves and what it does not: layout approval need not establish approval of white stock. Describe the scope of the new evidence rather than treating any approval as permission for every production choice.
Questions, answered
Does finding a wording change prove who caused the error?
No. The supplied versions identify a divergence, but authorship and intent require additional evidence.
Should the correction erase the earlier request?
Preserve the earlier records and mark which details the replacement supersedes, so the trail remains understandable.
